Who we’re looking for
We are looking for a part-time Youth Care Coordinator, with a minimum Certificate IV qualification in either Mental Health, Social Work, or Psychology to provide needed support to children, youth, and their families. If you enjoy variety in your day and working closely with other service delivery professionals, you might be exactly the person we're looking for!

As an important part of our Family Engagement Team in Toowoomba, EACH is looking for a Youth Care Coordinator to provide care coordination to children, youth, and their carers and families within the Darling Downs and West Moreton catchments.

Extensive experience supporting individuals with severe and complex needs is essential, and previous experience working with the step up step down model highly regarded.

What you’ll be doing
The Care Coordinator acts as the primary contact for children, young people, and their families who are, or are at risk of, experiencing a severe and/or complex mental health condition.  Each day the Care Coordinators assist and support young people and their families as they build capacity to manage their mental health concerns and navigate the mental health system.

You will make a huge difference in the lives of those navigating the mental health system as you use your extensive network and knowledge of mental health services within the Toowoomba region to connect clients to support services.

Who you’ll be working with
As an important member of this small team, you will work under the direction of the Clinical Lead and alongside other health professionals to provide youth-friendly mental health services within the Toowoomba region.
