As part of our transdisciplinary team in Southern NSW, EACH is looking for a Care Coordinator to work with families with children 0 - 6 years old with a developmental delay and make recommendations about the most appropriate supports to help their child achieve their goals making a huge difference to the lives of those with developmental delays and disabilities.

You will have experience working with families with children 0 - 6 with developmental delay and excellent knowledge of the sector. You will have qualifications within the early childhood sector, allied health or community sector.

What you’ll be doing
The Coordinator will work with families to assess the child's needs. This may include connecting to local specialists and mainstream services, short to medium-term early intervention supports and, where a child requires longer-term specialised early intervention, assist families to apply for, and then use a funded plan. This role will include meeting with the people best placed to influence a child’s physical and emotional wellbeing, preparing reports, documenting their progress, and keeping accurate records in accordance with NDIS requirements. Ultimately, you will be giving children and families skills to put therapies in place for day to day living.
