Who we're looking for
We're looking for a Home Care Package Case Manager, qualified as a Nurse DIV 1 who is motivated, clinically experienced in home care service delivery and capable of effectively engaging individuals and families to build positive and sustainable relationships, in a person-centred approach. Your qualifications will include DIV 1 Nursing and experience in Home Care Case Management. The role can be adjusted for full time or part-time work depending on your preference.
The successful applicant will need to draw upon their skills to build strong relationships with colleagues, local government, service networks and a range of community agencies.
What you'll be doing
The role of the Home Care Package Case Manager is to provide customers, their carers and families with enhanced access to holistic supports to maintain independent living according to preferences. Home Care Package Case Managers at EACH are expected to work collaboratively with the customer and their support network to monitor and review the individual plan, ensuring it remains responsive to changing needs and circumstances and within an agreed budget plan. Currently we are working remotely, however post COVID-19 restrictions it is expected that here will be a combination of office-based and home-site based work required to best serve the customers. You'll be supporting clients by connecting them with professionals to assist them with occupational therapy needs, home equipment requirements, domestic assistance or community connections depending on their needs and their plan.
Who you’ll be working with
Working with other professionals within the Home care Packages team, you will be one of 4 well experienced Case Managers in total, but the only Clinical Case Manger. The team report to a dedicated Team Leader focused on supporting the team and to giving clients the best possible care.
